Individual Approval Request Form

Students who would like to participate in an off-campus study program that has not been pre-approved by F&M must submit a formal request for individual approval. Students should also meet with an off-campus study adviser to discuss their interest. Deadlines for submitting individual approval requests are: October 1 (for study spring 2013) March 1 (for summer or fall 2013)
