Off-Campus Study Confirmation Form

All students who are planning to study off-campus for the upcoming semester must complete this form. Information from this form is sent to the Housing, Financial Aid and Business Offices. Please note that many off-campus study programs will also have their own confirmation materials. Please be sure to complete all required documents (from F&M and your program provider) by stated deadlines. NOTE: All fields are required.
